All clients are required to register and schedule via the on-line scheduling program.
(This is the program where your live video appointments / telehealth / tele-therapy will occur.)
Everyone can access the scheduling site and schedule appointments 24 hours a day once they register as a client.

*I do not do free meet and greet meetings or phone calls. What we both need to know will be discussed in the intake. If we are not a good match, I am happy to refer you to other providers.

You must be registered to see the appointments available.
On the scheduling site when the scheduling site will not let you select that appointment - please email me or text my office number, your name and what open time slot you would like.
I will put you in the appointment slot.
You do not have that appointment until you receive a confirmation text from me.
You cannot register and select an appointment for today as you must register, receive docusign documents and return them 24 hours prior to the first appointment.

You can request to reset your password information. You must be use the email address registered in the scheduling program for me to respond.
EACH REGISTRATION MUST BE IN ONE NAME ONLY AS IT IS A MEDICAL FILE.
PLEASE PUT YOUR SPOUSE / PARTNER who is participating under your emergency contact.
For individual sessions - put whomever you want in your emergency contact, YOU MUST HAVE AN EMERGENCY CONTACT.
WHEN SCHEDULING A TEEN- THE REGISTRATION IS IN THE TEEN'S NAME. THE FIRST APPOINTMENT WILL BE FOR THE PARENT(S) ONLY.
ADULTS MUST REGISTER THEMSELVES. DO NOT REGISTER YOUR SPOUSE OR ADULT PARTNER OR INDEPENDENT ADULT CHILD - ONLY REGISTER YOURSELF OR YOUR TEEN.
RE: FORMS
1) You will fill out the on-line registration and schedule your first appointment - Please enter all of your insurance information if you are seeking therapy / counseling.
YOU MUST ENTER AN EMAIL ADDRESS.
Icloud does not accept the size of my forms and you must use a different email address for the forms and then you can change to icloud for receiving messages re appointments and statements.
Please explain in your registration which service you want to schedule for.
2) You will receive an email to read.
3) You will receive an email from docusign with intake forms to complete within 48 hours from your registration time OR SOONER IF YOUR APPOINTMENT IS SOONER THAN 48 hours.
4) you will need to upload your ID and INSURANCE CARD into the registration / scheduling site. If you are unable to do this, please text your ID and the front and back of your insurance card to 253-446-9222 ASAP after you register.
